Sassy and Carter’s-branded Hug N’ Tug Baby Toys Recalled Due to Choking Hazard
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
Name of Products: Sassy(r)-branded Hug N’ Tug Puppy and Monkey and Carter’s(r)-branded Hug N’ Tug Monkey Toys
Units: About 45,300
Importer: Sassy, Inc., of Kentwood, Mich.
Hazard: The beads inside the clear plastic sphere at the center of the toys can be released and pose a choking hazard to young children.
Incidents/Injuries: The firm has received 12 reports of beads being released from the plastic clear plastic sphere body on Sassy-branded toys. There have been no such reports on the Carter’s-branded toys. No injuries have been reported.
Description: The recalled toys for infants can be attached to a stroller or car seat using a ring connector on the toy’s head. They have arms and legs that can be easily pulled back and forth for play. The toys are approximately 12 inches tall, made of multi-colored fabrics, contain rattle beads inside a clear plastic sphere body and have the face of either a puppy or monkey. Recalled styles include: Sassy Hug N’ Tug Puppy, model #80213; Sassy Hug N’ Tug Monkey model #80214 and Carter’s-branded Hug N’ Tug Monkey, models #61083 that were sold on a blue packaging card and #61540 sold on a grey packaging card. Sassy(r)-branded toys have “Sassy(r)” written on the sewn-in label located on the back of the character’s head. The Carter’s(r)-branded toys have “Carter’s(r)” written in red on the blue plastic ring connected to the toy’s head and also the sewn-in label located on the back of the character’s head.
Sold at: Mass merchandisers such as Toys R Us and Target, independent specialty stores nationwide and online retailers such as Amazon.com and Carters.com between July 2012 and October 2012 for Sassy-branded models; and from August 2011 to October 2012 for the Carter’s-branded models. Carter’s-branded models were also sold in Carter’s retail stores. The Sassy-branded models sold for approximately $8 and the Carter’s-branded models for approximately $14.

City Versa Strollers Recalled by Baby Jogger Due to Fall Hazard
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Name of Product: Baby Jogger City Versa(tm) strollers
Units: About 8,400 in the United States and 2,200 in Canada
Manufacturer: Baby Jogger LLC, of Richmond, Va.
Hazard: The stroller frame can fail to lock in place and collapse while in use, posing a fall hazard to children in the stroller.
Incidents/Injuries: Baby Jogger has received six reports of incidents with the stroller’s frame failing to lock. No injuries have been reported.
Description: This recall involves all Baby Jogger City Versa(tm) model strollers manufactured between May 15 and August 20, 2012. The date of manufacture is printed on the stroller’s black plastic side hinge below “Baby Jogger” and “City Versa.” “City Versa” is also printed on the stroller’s fabric in the child’s leg area. The strollers have charcoal-colored metal frames with black, silver, red or green fabric stroller seats and canopies.

Hazard: When two or more magnets are swallowed, they can link together inside a child’s intestines and clamp onto body tissues, causing intestinal obstructions, perforations, sepsis and death. Internal injury from magnets can pose serious lifelong health effects.
Incidents/Injuries: Reiss Innovations has received no reports of incidents or injuries. CPSC has received 80 reports of incidents involving ingestion of other high powered magnets, resulting in 79 reports seeking medical intervention.
Description: This recall involves high-powered magnet sets sold under the DynoCube.com brand name for use as a novelty item or desk toy. Each set contains 216 small, silver-colored, spherical magnets that are approximately 5 millimeters in diameter.
Sold exclusively at: Amazon.com/Dynocube.com from July 2011 through April 2012 for about $20.

Kolcraft Recalls Contours Tandem Strollers Due to Fall and Choking Hazards

Hazard: The front wheel assembly can break, posing a fall hazard to the child in the stroller. In addition, for strollers manufactured in January and February 2012, the nuts that hold the stroller’s basket support screws in place can detach. Detached nuts can pose a choking hazard to young children.
Incidents/Injuries: Kolcraft has received six reports of front caster wheels breaking and two reports of the basket’s support screws and nuts detaching. No injuries have been reported.
Description: This recall involves all Contours Options LT tandem strollers with model number ZT012. The model number and date of manufacture are printed on a label found on the rear leg of the stroller. The dual-seat strollers have one mesh basket beneath both seats and were sold in two color schemes; black with red canopies and accents, and gray with yellow canopies and accents. “Options LT” is printed on a bar along the side of the stroller.
Sold at: Burlington Coat Factory and juvenile product specialty stores nationwide and online at Amazon.com, Target.com and other online retailers between February 2012 and July 2012 for about $250.

Peg Perego Recalls Strollers Due to Risk of Entrapment and Strangulation; One Child Death Reported
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Peg Perego USA Inc., of Fort Wayne, Ind., is announcing a voluntary recall of about 223,000 strollers due to a risk of entrapment and strangulation.
A 6-month-old baby boy from Tarzana, Calif. died of strangulation after his head was trapped between the seat and the tray of his Peg Perego stroller in 2004. Another baby, a 7-month-old girl from New York, N.Y., nearly strangled when her head became trapped between the seat and the tray of her stroller in 2006.
Entrapment and strangulation can occur, especially to infants younger than 12 months of age, when a child is not harnessed. An infant can pass through the opening between the stroller tray and seat bottom, but his/her head and neck can become entrapped by the tray. Infants who become entrapped at the neck are at risk of strangulation.
The recall involves two different older versions of the Peg Perego strollers, Venezia and Pliko-P3, manufactured between January 2004 and September 2007, in a variety of colors. They were manufactured prior to the existence of the January 2008 voluntary industry standard which addresses the height of the opening between the stroller’s tray and the seat bottom. The voluntary standard requires larger stroller openings that prevent infant entrapment and strangulation hazards.
Only strollers that have a child tray with one cup holder are part of this recall. Strollers with a bumper bar in front of the child or a tray with two cup holders are not included in this recall.
The following Venezia and Pliko-P3 stroller model numbers that begin with the following numbers are included in this recall. The model number is printed on a white label on the back of the Pliko P-3′s stroller seat and on the Venezia stroller’s footboard.

Consumers should immediately stop using the recalled strollers and contact the firm for a free repair kit. Do not return the stroller to the retailers as they will not be able to provide the repair kit.

Strollers Recalled by Kolcraft Due to Fingertip Amputation and Laceration Hazards

Hazard: A child or consumer’s finger can become caught in the opening formed when locking and unlocking the hinge mechanism which is used to adjust the handlebars on the strollers. This presents an amputation and laceration hazard to children and the adults handling the stroller.
Incidents/Injuries: Kolcraft and CPSC have received five reports of injuries involving the hinge mechanism, including reports of three children whose fingertips were amputated and two adults whose fingers were either smashed or lacerated.
Description: This recall involves Kolcraft Contours Options three- and four-wheeled strollers. Strollers included in the recall have model numbers starting with ZL002, ZL005, ZL008, ZL015 and ZL018. On the ZL002 model, the model number and date of manufacture is printed on a sticker above the left wheel. On the ZL005, ZL008, ZL015 and ZL018 models, the model number and date of manufacture is printed on a label sewn into the edge of back of the stroller seat pad. The strollers were manufactured from January 2006 through November 2009 and sold in various color schemes.

The words “Kolcraft Tender Vibes” or “Kolcraft Light Vibes” is located on the removable music box which is attached to the side of the bassinet.

Hazard: The cribs’ drop sides can malfunction, detach or otherwise fail, causing part of the drop side to fall out of position, creating a space into which an infant or toddler can roll and become wedged or entrapped, which can lead to strangulation or suffocation. A child can also fall out of the crib. Drop-side incidents can also occur due to incorrect assembly and with age-related wear and tear.
Incidents/Injuries: CPSC and the firms are aware of five incidents involving drop sides that malfunctioned or detached, including one report of a child who became entrapped and sustained minor scratches and bruises.
Description: This recall includes Rockland Furniture brand drop-side cribs imported and sold by jcpenney from 2005 to 2008. The wooden cribs used plastic hardware to attach the drop side. “Rockland Furniture” and the model number can be found on a rectangular label located on the lower portion of the headboard panel. Important Message from CPSC: CPSC reminds parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for parts separating that can create a gap and entrap a child. In addition, do not try to repair any side of the crib. Babies have died in cribs where repairs were attempted by caregivers. Crib age is a factor in safety. At a minimum, CPSC staff recommends that you do not use a crib that is older than 10 years old. Effective June 28, 2011, new, mandatory federal crib rules require that all cribs manufactured and sold after that date must meet new and improved safety requirements (pdf). IKEA Recalls to Repair High Chairs Due to Fall Hazard
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
Name of Product: ANTILOP High Chairs
Units: About 169,000 (133,000 in the U.S. and 36,000 in Canada)
Importer: IKEA North America Services LLC, of Conshohocken, Pa.
Hazard: The high chair’s restraint buckle can open unexpectedly, posing a fall hazard to the child.
Incidents/Injuries: IKEA has received eight reports worldwide of restraint buckles that opened unexpectedly, including three reports of children who received minor injuries after falling from the high chair.
Description: This recall involves ANTILOP high chairs sold in red, blue or white. The plastic high chair has detachable silver-colored metal legs. High chairs included in the recall have a manufacture date between 0607 and 0911 (YYMM format) from supplier number 17389. The production date and supplier number are molded into the underside of the seat. “ANTILOP,” “IKEA” and the model number are printed on a label affixed to the underside of the seat. Model numbers included in the recall are:
ANTILOP high chair blue — Model # 701.467.92
ANTILOP high chair red — Model # 501.467.93
ANTILOP high chair white — Model # 300.697.24
Sold exclusively at: IKEA stores nationwide from August 2006 through January 2010 for about $20.
Manufactured in: China
Remedy: Consumers should immediately stop using the high chairs and contact IKEA to obtain a free replacement seat restraint.
