This is a mandatory form! All crib recipients must sign this form. This form states that the individual has accepted receipt of the crib and other products of his/her own free will; that he/she understands the safe sleep messages shared with him/her; that he/she intends to use the crib to safely sleep the baby; and, finally, that he/she will not hold First Candle or its agents (including you and your agency!) responsible for any issues that may arise with or as a result of the donated items.
