STEP 1: Do the Paperwork

To launch your role in this program’s evaluation, first begin by completing the Distribution Agency Agreement and the Confidentiality Agreements and returning them directly to your Site Director.

Then, you and any of your staff that will be administering surveys may be required to take additional training in human subjects protection before you become involved in the evaluation and the informed consent process. Participation in such training was included in our submission to the various IRBs. Your Site Director will let you know the training requirements.

You may access First Candle’s Informed Consent Process training on the secure section of the Bedtime Basics for Babies website under Online Training. Informed Consent is a critical component of this evaluation, designed to protect the rights and well-being of your crib recipients and our evaluation participants.

STEP 2: Order Your Supplies

In addition to the Bedtime Basics Kits, you will need a supply of the educational brochures, posters, surveys and requisite forms on-hand before you begin outreach to potential recipients.

Visit the online store to download or order the educational materials, surveys, Informed Consent Forms and all other materials associated with this campaign.

STEP 3: Determine Eligibility

Upon your first meeting with a potential crib recipient, be sure that the person meets the eligibility guidelines outlined by this program, namely:

Your Site Coalition has identified specific eligibility guidelines that will be applied across your state/site. Please contact your Site Director if you do not know these particular guidelines.

Serving Minors

This program provides cribs and educational materials to all minors, regardless of emancipation and guardian approval. Only emancipated minors may participate in the evaluation.

First Candle’s National Crib Campaign does not require proof of residence, pregnancy/recent birth or financial need. This remains the responsibility of the distributing agency.

STEP 4: Discuss the Evaluation and Obtain Informed Consent

Begin by discussing with your qualified crib recipient the importance of the evaluation and employ all of the techniques that you learned previously in the Informed Consent Process training.

If possible, show the potential crib recipient the first segment of the video, where Dr. Moon discusses Informed Consent.

If the crib recipient determines openly and freely that she or he would like to participate in the evaluation, have her sign the Informed Consent Form. This is mandatory for all individuals who elect to participate in the evaluation. However, it is important to note that individuals who elect not to participate in the evaluation can still receive a crib!

Submit the signed Informed Consent Forms for receipt by the fifth day of each month to your Site Director.

STEP 5: Complete the Crib Receipt, Release and Waiver of Liability

Once again, please review this document with the recipient so that she is aware of and understands what we’re asking her to sign. Submit all signed Waivers for receipt by the fifth day of each month to your Site Director.

STEP 6: Administer the Pre-Survey

The Pre-Survey is designed to learn about mom, her pregnancy and her infant care practices. We have developed two Pre-Surveys, one for prenatal mothers and one for mothers who have already given birth.

ONLY THOSE RECIPIENTS WHO HAVE SIGNED THE INFORMED CONSENT FORMS SHOULD COMPLETE THE PRE-SURVEY.

You may use either a pen or pencil to complete this survey. Please fill in the bubble marks as completely as you can and avoid stray pen or pencil marks on this survey. For write-in responses, please print as legibly as you can.

Submit completed Pre-Surveys for receipt by the fifth day of each month to your Site Director. Alternatively, you may elect to complete the survey online via the secure section of the Bedtime Basics for Babies website under On-Line Surveys.

STEP 7: Educate the Parent or Caretakers

Share with your crib recipient the basics on safe sleep recommendations, as outlined in the Bedtime Basics for Babies brochure. It is optimum if you walk through the brochure with them, being sure that they understand the key points.

Also, show your crib recipient the remaining segments of the video, which provide an overview on how to set-up the portable crib and the vignettes on safe sleep.

STEP 8: Administer Either the Home Visit Survey or the Post-Survey

Our Evaluation Team has designed two different survey tools to capture important information on whether or not (and how) a mother or caregiver is putting the risk reduction recommendations and Bedtime Basics Kit to use. These are the Home Visit Survey and the Post-Survey.

If possible, please complete the Home Visit Survey rather than the Post-Survey.

The Home Visit Survey

If your program provides a home visiting service, this survey is designed to be administered by a visiting nurse, caseworker or home visitor who may be visiting the baby’s home. If you do provide a home visiting service, this survey may take the place of the Post-Survey.

The Post-Survey

The Post-Survey is designed to learn about how the mother and/or other caregivers are actually caring for their infants. For parents who receive the portable crib prior to the birth of their baby, we would like you to complete this survey when the infant is between 1 and 3 months of age. USE THIS SURVEY ONLY IF YOU DO NOT COMPLETE A HOME VISIT SURVEY.

ONLY THOSE RECIPIENTS WHO HAVE SIGNED THE INFORMED CONSENT FORMS SHOULD COMPLETE EITHER THE HOME VISIT OR POST-SURVEYS.

STEP 9: Complete Your Monthly Report

As a crib distribution site, you will be required to submit a monthly report, via First Candle’s web-based database. The report has two key components:

Crib Distribution

The first component is simply a database that will capture the name and contact information for everyone who receives a crib through our program, regardless of participation in the evaluation. This information will be kept in a secure database and will not be tied to any answers they may provide as part of the evaluation. We are collecting this data only if we need to contact one of the recipients in the future, but we would not do so without your approval and involvement.
